Job summary

Innovive Health is seeking a compassionate Occupational Therapist to join our Home Health team. You will evaluate patients in their homes, develop treatment plans, and deliver evidence-based interventions to improve independence and safety.

This role requires strong clinical judgment, excellent communication, and a patient-centered approach to care in a home health setting. Travel across the service area is expected.

Qualifications

• Graduate degree in Occupational Therapy from an accredited program.
• Current and unrestricted Massachusetts Occupational Therapist license.
• Current CPR certification.
• Minimum of 1 year of experience in home health, skilled nursing, acute care, or inpatient rehabilitation.
• Valid driver's license and ability to travel within the service area.

Responsibilities

• Perform home-based OT evaluations and follow-up visits.
• Develop and adjust individualized treatment plans with the care team.
• Set therapy goals and communicate progress to physicians and caregivers.
• Provide ADL training, mobility, neuromuscular re-education, and therapeutic exercise.
• Educate patients and families on home programs and equipment use.
• Maintain compliant EMR documentation and coordinate DME needs.
• Collaborate with interdisciplinary team to support discharge planning.
