Job summary

Innovive Health is seeking a Licensed Physical Therapist to join our in-home health team in Massachusetts. You will deliver evaluations, start of care visits, follow-ups, and discharges, while developing tailored treatment plans and educating patients and caregivers on safety and mobility.

Qualifications

• Must be a Licensed Physical Therapist in Massachusetts.
• CPR certification current.
• Valid driver’s license and reliable transportation.
• Strong communication, organization, and clinical decision-making skills.
• Home health experience required.

Responsibilities

• Perform in-home PT evaluations, start of care visits, follow-ups, recertifications, and discharges.
• Develop and implement personalized treatment plans.
• Lead gait training, mobility, balance activities, and therapeutic exercises.
• Educate patients and caregivers on safety, fall prevention and daily living strategies.
• Monitor progress and adjust plans for optimal outcomes.
• Assess vital signs as needed and communicate concerns to the care team.
• Recommend and educate on use of assistive devices and DME as appropriate.
• Collaborate with nurses, OTs, STs, and physicians.
• Complete timely and accurate documentation.
• Support discharge planning and continuity of care.
