Iowa Home Care in Webster City, IA is seeking a compassionate Physical Therapist Assistant to provide skilled in-home therapy under a licensed PT. You will help patients improve mobility, safety, and independence while collaborating with a multidisciplinary team and caregivers.

This role supports in-home visits, flexible scheduling, and a mission to keep patients safely at home with quality care and ongoing professional development.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Iowa PTA license.
• CPR certification is current.
• Valid driver's license with reliable transportation and auto insurance.

Responsibilities

• Provide skilled physical therapy treatments in patients' homes under a licensed PT.
• Implement individualized treatment plans to improve mobility, strength and balance.
• Assist patients with therapeutic exercises, gait and transfer training.
• Instruct patients and caregivers on safe home exercise programs.
• Document patient responses and progress toward goals.
• Communicate changes in condition to the supervising PT and care team.
• Recommend adaptive equipment and home safety modifications as needed.
• Maintain accurate documentation per agency policies and regulations.
• Coordinate care with nurses, therapists, social workers and case managers.
• Promote patient independence and quality of life with compassionate care.
