Job summary

Advanced Home Health Care in Keokuk, IA is seeking a Physical Therapist Assistant to provide home-based PT services. The role requires collaboration with the Physical Therapist and RN Case Manager to implement prescribed treatment plans.

The PA will visit clients in their homes, document progress, participate in team trainings, and educate patients and families while adhering to agency policies.

Qualifications

• Must possess PTA license from an accredited college or university.
• Must be able to perform duties in client homes.
• Must have a safe, dependable vehicle and Iowa driver’s license.
• Current CPR certification.
• Current Dependent and Child Abuse certification.
• A physical exam and TB skin testing are required.

Responsibilities

• Be responsible to the Physical Therapist/AHHC RN Case Manager.
• Provide home-visit physical therapy in the geographic area assigned by AHHC.
• Check patient charts prior to treatment.
• Follow prescribed treatment program as directed by the Physical Therapist and physician.
• Notify the Physical Therapist of any change needing re-evaluation before the next co-treatment.
• Document daily progress records.
• Participate in in-services, education programs, and staff meetings.
• Direct clinic care questions to the Physical Therapist/AHHC RN Case Manager.
• Educate patient and family.
• Perform duties as designated by the Physical Therapist.
