Job summary

Amazing Care Home Health Services is seeking a Speech Language Pathologist to join our Utah County team. The SLP will implement treatment plans in home and community settings to support clients of all ages with cognitive, speech, language, feeding/swallowing, and social/emotional needs.

Emphasis on collaboration with physicians and caregivers. The role requires a master’s or doctoral degree in SLP, Utah license, ASHA/PRAXIS qualification, and a valid BLS certificate; 1+ year experience

Qualifications

A master's or doctoral degree in speech-language pathology (SLP).
ASHA Certification or PRAXIS exam with Clinical Fellow/Externship completion.
Utah SLP license in good standing.
Active BLS certification.

Responsibilities

Perform evaluations and interpret assessment results; develop plans of care and speech-language treatment plans.
Assist pediatric and adult patients to improve cognitive, speech, language, feeding/swallowing, and social/emotional functioning.
Coordinate care with referring physicians and other health care professionals.
Instruct client and caregivers on treatment plans; refer for additional services as needed.
Prepare and submit documentation per policy; maintain clinical competency.
