Job summary

Amazing Care Home Health Services in Utah's Logan area is seeking a Speech Language Pathologist to deliver in-home and community-based therapy for clients of all ages. The SLP will implement treatment plans, conduct evaluations, and collaborate with physicians to create individualized goals and speech-language programs.

Requirements include a master's or doctoral degree in SLP, ASHA certification or PRAXIS/externship documentation, Utah SLP license, active BLS, and a valid driver's license.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology (SLP).
• ASHA Certification or documentation of PRAXIS exam and externship/CFY completion.
• Utah SLP License and active BLS certification.
• Valid driver's license and auto insurance.
• Minimum 1 year of clinical experience preferred.

Responsibilities

• Provides evaluations and develops speech-language treatment plans in collaboration with physicians.
• Instructs client, family, and caregivers in the treatment plan and home activities.
• Coordinates care with referring physicians and other health care providers.
• Identifies client needs for other services and makes appropriate referrals.
• Prepares and submits documentation per organization policy.
• Maintains clinical competency in the theory and practice of speech-language pathology.
