Connect Pediatrics is seeking a Pediatric Physical Therapist to join our team delivering in-home therapy in the Stigler, Oklahoma area. You will collaborate with physicians, nurses, and families to coordinate comprehensive care for medically fragile children.

The role emphasizes building lasting relationships with patients and families, providing structured evaluations, and creating and adjusting treatment plans in the home setting. A valid Oklahoma license and CPR/BLS are required.

Qualifications

• Valid, active Physical Therapist license in the state of Oklahoma.
• Current BLS/CPR Certification.
• Reliable transportation and valid driver's license with automotive insurance.
• Pediatric experience is preferred but not required.

Responsibilities

• Perform comprehensive pediatric evaluations using standardized tests to determine therapy needs.
• Develop and implement treatment plans aligned with physician orders.
• Provide in-home therapy services for pediatric patients with diverse diagnoses.
• Engage families with regular updates on goals and progress.
• Collaborate with a multidisciplinary team to create holistic care plans.
• Maintain thorough documentation and communicate changes to the care team.
• Participate in team meetings to discuss patient progress and refine strategies.
