Connect Pediatrics is seeking a Pediatric Physical Therapist to join our team in the Aurora, Colorado area. You will provide in-home therapy services for medically diverse pediatric patients, collaborating with physicians, nurses, and other clinicians to deliver coordinated care in the patients' homes.

We emphasize building lasting relationships with families, offering mentoring, and a supportive, electronic documentation workflow.

Qualifications

• Active Physical Therapist license in Colorado
• Current BLS/CPR certification
• Reliable transportation, valid driver's license and auto insurance
• Pediatric experience preferred but not required

Responsibilities

• Perform comprehensive pediatric evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Provide in-home therapy services for pediatric patients with diverse diagnoses.
• Engage with families, giving regular updates on goals and progress.
• Collaborate with a multidisciplinary team to create holistic treatment plans.
• Maintain comprehensive documentation and communicate changes to the care team.
• Participate in team meetings to discuss patient progress and refine strategies.
