Job summary

Allied Therapy Partners is seeking a dedicated Early Intervention Physical Therapist to join our Chicago-area team. This full-time role focuses on the Illinois EI program, serving infants and toddlers with diverse diagnoses in home and community settings.

You will design and implement PT plans, coach families on home programs, maintain timely documentation, and collaborate with caregivers and other professionals.

Qualifications

• Illinois Physical Therapist license required.

Responsibilities

• Intervention & Planning: Implement and carry out individualized Physical Therapy Plans of Care using evidence-based interventions.
• Family Coaching: Develop and instruct families on effective home exercise programs to ensure therapy carry-over.
• Documentation: Complete daily SOAP notes and required reports for insurance payers or EI program.
• Collaboration: Communicate with families, therapists, children, caregivers, and other professionals.
• Service Delivery: Conduct therapy sessions in home/community settings or via telehealth.
