Connect Pediatrics is seeking a Pediatric Physical Therapist to join our in-home care team serving the Denver, Colorado area. You will evaluate children, design and implement therapy plans, and collaborate with families and a multidisciplinary team to support each child's goals.

Formación

• Must have a valid Colorado Physical Therapist license.
• Current BLS/CPR certification.
• Reliable transportation, valid driver's license, and auto insurance.
• Pediatric experience is preferred but not required.

Responsabilidades

• Perform comprehensive evaluations, using standardized tests to determine pediatric patients' needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician's orders.
• Provide in-home therapy services for pediatric patients with a wide range of diagnoses.
• Engage with patients' families, providing them with regular communication and updates on goals for their child.
• Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient
• Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.
• Actively participate in one-on-one meetings with team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.
