Job summary

Circadia Health in Corona, CA is seeking a Patient Care Coordinator to join our home-based care team. You will travel to patient homes across Orange County to install remote monitoring devices, record vitals, and facilitate initial telehealth visits with our Nurse Practitioners, advancing proactive care.

You will support device maintenance, coordinate with internal teams, and help patients feel comfortable using the Circadia platform.

Qualifications

Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required.
Experience taking and recording vital signs (BP, pulse, respiration, temp, SpO2).
Valid driver's license and reliable transportation for daily travel between patient homes.
Ability to work independently with minimal supervision.
Strong communication skills with elderly or vulnerable populations.
Familiarity with HIPAA and patient privacy standards.
Willingness to pass a background check.
Bilingual Spanish a plus for the Orange County area.

Responsibilities

Take and record patient vitals during home visits and transmit readings.
Oversee installation of Circadia devices at patient locations for the Home Health Program.
Facilitate initial telehealth visits with our Home Health Nurse Practitioner.
Educate patients to use the device and set up telehealth visits.
Manage routine device maintenance and IT setup.
Coordinate with internal teams to ensure successful deployment.
Audit and maintain the asset management list.
Support the Procurement Team with supply chain planning for future installs and company growth.
Come into the office once a week, or as needed, to pick up supplies.
