Job summary

The Pennant Group, Inc. is hiring a Nurse Practitioner to provide in-home palliative and hospice care in the Yuma, AZ area. You will perform visits, assessments, and collaborate with physicians to tailor care plans that ensure comfort.

You will prescribe treatments, educate families, and guide hospice eligibility decisions while enjoying autonomy in scheduling and a team-driven, compassionate work environment.

Qualifications

• Master’s degree and completion of an accredited Nurse Practitioner program.
• Current state licensure and CPR certification.
• Certification in Hospice and Palliative Nursing (CHPN) or geriatrics/pediatrics preferred but not required.
• At least 1 year of NP experience and 1 year in hospice or palliative care.
• Strong communication, problem-solving, and clinical skills.

Responsibilities

• Conduct in-home visits, physical assessments, and face-to-face evaluations.
• Develop and manage individualized care plans with physicians and care teams.
• Prescribe medications and treatments for symptom control and comfort.
• Educate and support patients and families through end-of-life care decisions.
• Serve as a trusted resource for hospice eligibility and recertification.
