Job summary

The Pennant Group, Inc. is hiring a Nurse Practitioner to provide in-home palliative and hospice care in Arizona. You will conduct visits, assess patients, and collaborate with physicians to develop personalized care plans that ensure comfort and dignity for patients and families during end-of-life moments.

Requirements include an NP license, CPR, and a master’s degree. CHPN or related hospice specialization is preferred but not required; at least 1 year of NP and hospice experience is expected.

Qualifications

• Master’s degree and completion of an accredited Nurse Practitioner program
• Current state NP licensure and CPR certification
• CHPN or related hospice/palliative specialty preferred but not required
• At least 1 year NP experience and 1 year in hospice/palliative care
• Strong communication and clinical skills

Responsibilities

• Conduct in-home visits and assessments
• Develop and manage individualized care plans with physicians and care teams
• Prescribe medications and treatments for symptom control
• Educate and support patients and families in end-of-life decisions
• Serve as a trusted resource for hospice eligibility and recertification
