Qualifications

• Active RN license
• NP or PA license with board certification
• DEA certificate (or eligibility)
• 2+ years direct patient care experience
• Field-based travel to patient homes; reliable transportation and valid driver’s license

Responsibilities

• Provide evidence-based, patient-focused in-home care
• Perform comprehensive assessments including wellness exams and follow-ups
• Educate patients and caregivers on disease management and medications
• Participate in virtual integrated care team meetings
• Coordinate care with PCPs and specialists
• Complete documentation accurately and promptly
• Travel to patient homes as needed
