Job summary

VitalCaring Group seeks a Palliative Care Nurse Practitioner to provide in-home medical care for patients with serious illnesses in Mississippi. You will work independently in the field, delivering evidence-based care and serving as a clinical resource for patients and families.

The role emphasizes symptom management, advance care planning, and coordination with existing providers, with collaboration across home health and interdisciplinary teams.

Qualifications

• Master's or Doctoral degree in an accredited NP program.
• Current NP license in the state of practice.
• National certification through AANP or ANCC.

Responsibilities

• Conduct comprehensive in-home assessments and develop care plans.
• Manage symptoms including pain, dyspnea, nausea, and anxiety.
• Prescribe medications and order tests within scope of practice.
• Collaborate with physicians and interdisciplinary teams for coordinated care.
• Lead advance care planning discussions and patient education.
