Job summary

VitalCaring Group is hiring a Palliative Care Nurse Practitioner to deliver comprehensive in-home medical care for patients with serious illnesses. You will manage symptoms, plan care with families, and coordinate with interdisciplinary teams to align treatment with patient goals and values.

You will work independently in the field, utilizing evidence-based care to support patients, caregivers, and healthcare partners while ensuring documentation and regulatory compliance.

Qualifications

• Master's or Doctoral degree from an accredited Nurse Practitioner program.
• Current Nurse Practitioner license in the state of practice.
• National certification through AANP or ANCC.
• Current BLS certification.
• Valid driver's license and reliable transportation for home visits.
• Strong clinical assessment and communication skills.
• Ability to work independently while managing a mobile patient caseload.

Responsibilities

• Conduct comprehensive in-home assessments and develop individualized care plans.
• Evaluate and manage symptoms to optimize quality of life.
• Prescribe medications and order appropriate tests within scope of practice.
• Coordinate care with physicians, home health, hospice and interdisciplinary teams.
• Document care in the electronic medical record and ensure regulatory compliance.
• Participate in on-call responsibilities as needed.
