Job summary

Signify Health, Inc. is hiring board-certified PA contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plans. You will perform comprehensive health assessments in members' homes, review medical history, and ensure safe, effective care without prescribing or ordering labs.

Flexible schedule and travel incentives included. Pre-scheduled in-home visits with health plan members, malpractice coverage, and 1099 status are provided.

Qualifications

• Board-certified PA contractor to conduct in-home evaluations.
• State license required (PA-C) and ability to work as an independent contractor.
• Experience performing health assessments and reviews in patients' homes.

Responsibilities

• Conduct in-home visits focused on overall health and barriers to care.
• Pre-scheduled visits with health plan members in their homes.
• Review medications and medical history; perform physical evaluation.
• Do not prescribe medications or order labs; manage clinical commitments.
