Job summary

Signify Health, Inc. is hiring board-certified PA contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and select commercial members. You will perform in-home visits focused on overall health, review medical history, and determine barriers to care.

Shifts are available Monday through Sunday with no nights or weekends required. Earn up to $1,800–$3,000 per week, with a base rate of $70 per completed assessment and potential increases via a market multiplier.

Qualifications

• Board-certified PA or physician assistant with active license or eligibility.
• Eligibility or enrollment in Medicaid preferred or supported by Signify Health.
• Ability to perform in-home health evaluations and document findings.

Responsibilities

• Conduct in-home visits focused on overall health and barriers to care.
• Review medical history and perform a physical evaluation in member's home.
• Provide pre-populated forms for each evaluation and avoid prescribing or ordering tests.
