Job summary

Signify Health, Inc. is hiring board-certified PA/physician associate contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and some commercial plan members. You will perform comprehensive health reviews in members' homes and document findings for care planning.

Shifts run Monday through Sunday with no nights or weekends required. This contractor role offers a flexible schedule and includes malpractice coverage, travel incentives, and potential for additional pay

Qualifications

• Board-certified PA/Physician Assistant
• In-home health evaluation experience preferred
• Must be able to conduct home visits without prescribing or ordering tests

Responsibilities

• Conduct in-home visits focusing on overall health and identifying barriers to care
• Pre-scheduled home visits with health plan members
• No prescribing medications, ordering labs, or changing current treatments during evaluation
