Job summary

Creative Solutions Services, LLC is seeking a Physical Therapist (PT) for an in-home/outpatient rehabilitation role based in Siler City, NC. The position is onsite with a fixed 18-week contract starting 9/28/2026, offering 8 hours per day, 5 days per week.

The PT will evaluate and treat in patients' homes or in independent living facilities, developing individualized therapy plans to restore and enhance function.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy.
• Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy.
• Bachelor's degree in Physical Therapy plus 5 years of progressive PT experience.
• Degrees earned outside the U.S. must be evaluated for U.S. equivalence.
• Valid driver's license or reliable transportation to patients' homes.
• Excellent organizational and time-management skills.
• Good verbal and written communication skills.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as needed.
• Adhere to department and patient treatment schedules and complete documentation.
• Attend and contribute to patient care and staffing conferences.
• Make referrals to other disciplines to address patient needs.
• Collaborate with rehabilitation team to select appropriate therapeutic regimens.
• Adapt plans of care based on patient progress and needs.
• Promote safe environment and manage equipment and supplies.

Education

Master's degree in Physical Therapy

Master's degree in Physical Therapy related field + Bachelor's in PT

Bachelor's degree in Physical Therapy + 5 years PT experience

Licensed Physical Therapist in-state
