Job summary

Aspire Home Healthcare Inc in San Diego, CA is seeking a Licensed Occupational Therapist (OT) to deliver home health services across the North County Coastal/Inland area. You’ll evaluate patients, design individualized therapy plans, and help them regain independence in their home.

This full/part-time OT role offers competitive per-visit pay, mileage reimbursement, and benefits for eligible employees, including PTO and bonuses.

Qualifications

• Licensed OT in CA.
• Current TB test (within last 12 months).
• CPR certification.
• Current physical (within last 6 months).
• Valid driver’s license and reliable transportation.
• 2–3 day orientation at our San Diego office.

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patients’ condition, function level, and treatment goals.
• Develop individualized treatment plans and update as needed.
• Supervise COTAs and HHAs per care plan requirements.
• Document treatment notes and progress in the patient’s chart daily.
• Assist patients and families with home exercise programs and adaptive equipment needs.
• Coordinate total patient care with the healthcare team.
• Complete discharge summaries and obtain orders when needed.
