Job summary

Aspire Home Healthcare Inc. in San Diego, CA seeks an Occupational Therapist (OT) for full- or part-time home health. You will evaluate and treat patients at home, designing therapy plans to help regain independence and coordinating with the rehab team.

Requirements include CA OT license, 1 year of OT experience preferred, and experience with OASIS/WellSky. Current TB and CPR, a valid driver’s license, and willingness to travel within North County Coastal/Inland are required.

Qualifications

• Licensed Occupational Therapist (OT) in CA.
• Minimum 1 year OT experience preferred.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Current physical (within the last 6 months).
• Current TB test (within last 12 months) and CPR certification.
• Valid driver's license and reliable transportation.
• Ability to attend a 2-3 day orientation at our San Diego office.

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patients' condition, function level, and treatment goals.
• Develop individualized treatment plans and update as needed.
• Supervise COTAs and HHAs per care plan requirements.
• Document treatment notes and progress in the patient's chart daily.
• Assist patients and families with home exercise programs and adaptive equipment needs.
• Recommend and evaluate adaptive equipment for the home setting.
• Maintain compliance with HIPAA regulations and company policies.
• Coordinate total patient care with the healthcare team.
• Complete discharge summaries and obtain orders when needed.
• Submit patient documentation and visit records in a timely manner.
