Job summary

Granite Peak Home Health, based in Butte, Montana, seeks a licensed Occupational Therapist to provide in-home therapy for adults and geriatric patients in a home health setting.

You will perform evaluations, develop treatment plans, and deliver skilled services in patients' homes, aiming to improve ADLs/IADLs and independence. Collaboration with an interdisciplinary team and compliant EMR documentation are essential.

Qualifications

• Active MT OT license and degree from an accredited OT program.
• Minimum 1 year of home health, skilled nursing, rehab, or acute care experience preferred.
• Knowledge of Medicare guidelines and OASIS documentation preferred.
• Strong skills in patient assessment, care planning and clinical decision-making.
• Valid driver’s license and ability to travel between patient homes.
• Current CPR/BLS certification.

Responsibilities

• Perform occupational therapy evaluations and OASIS assessments if applicable.
• Develop and implement individualized treatment plans.
• Provide skilled therapy services in patients’ homes.
• Improve patients’ ability to perform ADLs and IADLs.
• Recommend adaptive equipment, home modifications and fall prevention strategies.
• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers).
• Complete accurate EMR documentation in compliance with Medicare and home health regulations.
• Monitor patient progress and adjust care plans accordingly.

Position Summary

We are seeking a licensed Occupational Therapist (OT) for a home health position. This role focuses on adult and geriatric patient care, providing in-home therapy services to individuals recovering from illness, injury, or surgery.

This is a great opportunity for an OT looking for flexible scheduling, autonomy, and meaningful one-on-one patient care in a home health setting.

Key Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable)

• Develop and implement individualized treatment plans

• Provide skilled therapy services in patients’ homes

• Improve patients’ ability to perform activities of daily living (ADLs) and instrumental ADLs (IADLs)

• Recommend adaptive equipment, home modifications, and fall prevention strategies

• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers)

• Complete accurate EMR documentation in compliance with Medicare and home health regulations
