Job summary

Granite Peak Home Health in Polson, Montana seeks a licensed Occupational Therapist to deliver in-home therapy to adults and geriatrics, focusing on improving ADLs and IADLs. This PRN role offers autonomy, flexible scheduling, and meaningful one-to-one patient care in a home health setting.

You will perform evaluations, develop plans, document in EMR per Medicare rules, and travel to patient homes in Polson and surrounding areas.

Qualifications

• Active Montana OT license.
• Degree from an accredited OT program.
• Minimum 1 year experience in home health, skilled nursing, rehab, or acute care (preferred).
• Knowledge of Medicare guidelines, OASIS documentation, and home health compliance (preferred).
• Strong skills in patient assessment, care planning, and clinical decision-making.
• Valid driver’s license and ability to travel between patient homes.
• Current CPR/BLS certification.

Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable).
• Develop and implement individualized treatment plans.
• Provide skilled therapy services in patients’ homes.
• Improve patients’ ability to perform ADLs and IADLs.
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers).
• Complete accurate EMR documentation in compliance with Medicare and home health regulations.
• Monitor patient progress and adjust care plans accordingly.

Position Summary

We are seeking a licensed Occupational Therapist (OT) for a home health position. This role focuses on adult and geriatric patient care, providing in-home therapy services to individuals recovering from illness, injury, or surgery.

Key Responsibilities

• Perform occupational therapy evaluations and OASIS assessments (if applicable)
