Job summary

Jukebox Health is seeking an Occupational Therapist specializing in seating, positioning, and mobility for in-home evaluations. You will assess wheelchair needs, recommend DME and seating solutions, and collaborate with families to create safer home environments.

The role requires hands-on experience with wheelchair evaluations and DME, with a focus on aging in place and accessibility. This is a contracted position with weekly availability for in-home visits across Massachusetts.

Qualifications

• Must have OT degree and active OT license.
• Minimum 2+ years of clinical OT experience.
• Specialized seating and wheeled mobility training (ATP/SMS) or equivalent experience.
• Experience with wheelchair evaluations, seating/positioning, and DME recommendations.

Responsibilities

• Conduct in-home wheelchair evaluations and seating/positioning assessments.
• Assess clients for DME, mobility equipment, and adaptive seating solutions.
• Deliver evidence-based recommendations for home modifications and mobility equipment.
• Educate clients and caregivers on fall prevention, safe transfers, and mobility techniques.
• Engage clients and families in collaborative care planning.
• Share findings with the team and support process improvement initiatives.
• Maintain accurate documentation in the electronic system with seating/ mobility justifications.
