Job summary

Jukebox Health is seeking an Occupational Therapist to conduct in-home wheelchair evaluations and seating/positioning assessments. You will assess clients for DME, mobility equipment, and adaptive seating solutions, delivering personalized recommendations for home modifications and mobility devices.

Candidate must have OT degree, active license, 2+ years of clinical experience, and specialized seating/wheeled mobility training.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Specialized training or certification in seating and wheeled mobility (e.g. ATP, SMS) required.
• Experience with wheelchair evaluations, seating/positioning assessments, and DME recommendations.

Responsibilities

• Conduct in-home wheelchair evaluations and seating/positioning assessments.
• Assess clients for appropriate DME, mobility equipment, and adaptive seating solutions.
• Deliver personalized, evidence-based recommendations for home modifications and mobility equipment.
• Educate clients and caregivers on fall prevention, safe transfers, and mobility techniques.
• Engage clients and families in collaborative care planning.
• Work closely with our team to share findings and support process improvement initiatives.
• Maintain accurate and timely documentation through our electronic system, including detailed seating and mobility justifications.
