Qualifications

• OT degree from an accredited program
• Active, unrestricted OT license
• 2+ years of clinical experience
• Specialized seating and wheeled mobility training/certification
• Experience with home health or aging in place

Responsibilities

• Conduct in-home wheelchair evaluations and seating/positioning assessments
• Assess clients for DME, mobility equipment, and adaptive seating solutions
• Provide evidence-based recommendations for home modifications and mobility equipment
• Educate clients and caregivers on fall prevention, safe transfers, and mobility techniques
• Engage clients and families in collaborative care planning
• Work closely with our team to share findings and support process improvement initiatives
• Maintain accurate and timely documentation through our electronic system, including detailed seating and mobility justifications
