Job summary

FOX Rehabilitation is expanding in Franklin, MA, seeking licensed Occupational Therapists to deliver individualized care to older adults in private residences or Senior Living Communities as part of our Geriatric House Calls model.

We offer autonomy, flexible scheduling, and opportunities to incorporate telehealth, with a collaborative, interdisciplinary team to support patient outcomes and professional growth.

Qualifications

• Licensed OT in state(s) of practice or eligible to obtain license.
• Degree from accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification and refresh as required.

Responsibilities

• Provide one-on-one occupational therapy to older adults in private residences or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence, and safety.
• Document activities in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers and staff to promote aging in place and safety.
• Offer telehealth/virtual care when appropriate to extend access.
