Interim HealthCare of Queen Creek is seeking a part-time Home Health Occupational Therapist to evaluate patients in Queen Creek and surrounding East Valley communities. You will design and implement therapy plans focusing on independence in daily activities within the patient’s home.

You will collaborate with a multidisciplinary team, adapt interventions to home environments, and educate patients and caregivers. Arizona OT licensure and CPR certification are required.

Qualifikationen

• Graduate of an accredited occupational therapy program.
• Active Arizona OT license.
• CPR certification.
• Home health experience preferred.

Aufgaben

• Evaluate functional abilities, routines, safety risks, and barriers in the home.
• Develop and update individualized therapy plans per physician orders and patient goals.
• Provide interventions to improve strength, coordination, endurance, cognition, and daily living skills.
• Teach adaptive techniques for bathing, dressing, grooming, and meal prep.
• Recommend adaptive equipment and home environmental modifications.
• Educate patients and caregivers on exercises and plan of care.
• Track progress, adjust interventions, and document timely updates.
• Collaborate with nursing, PT, and speech therapy teams.
