Job summary

Interim HealthCare of Queen Creek is seeking a Home Health Occupational Therapist to assess patients in their own homes and design personalized plans that promote independence and safe daily activity performance.

You will deliver client-centered therapy, adapt environments, and educate families while collaborating with the care team to ensure quality outcomes. This role offers flexible scheduling and meaningful, in-home impact.

Qualifications

• Graduate of an accredited occupational therapy program with an active Arizona OT license.
• Must hold an active Arizona OT license to practice.
• Home health experience is preferred but not required.

Responsibilities

• Evaluate functional abilities, routines, safety risks, and barriers within the home.
• Develop and update individualized therapy plans aligned with physician orders and patient goals.
• Provide interventions to improve strength, coordination, endurance, cognition, and daily living skills.
• Teach adaptive techniques for bathing, dressing, grooming, meals, and daily activities.
• Recommend adaptive equipment and environmental modifications to support safety and independence.
• Educate patients and caregivers on exercises, strategies, equipment, and plan of care.
• Track progress, adjust interventions, and maintain timely documentation.
• Collaborate with nursing, physical therapy, speech therapy, and other care team members.

What You Bring

• Graduate of an accredited occupational therapy program with an active Arizona OT license
