Job summary

Home Health Care, Inc. in Minnesota seeks a dedicated Occupational Therapist (OTR/L) to serve patients in their homes across the Twin Cities Metro Area. This full-time role emphasizes helping clients regain independence through personalized therapy plans.

You will perform home evaluations, create goals to improve daily living, assess safety, educate patients and caregivers, and collaborate with a multidisciplinary team. A Minnesota license and valid driver’s license are required.

Qualifications

• Active Minnesota OTR/L license required.
• Home health experience is preferred but not required.
• Strong communication and organizational skills.
• Valid driver\'s license and reliable transportation.

Responsibilities

• Perform occupational therapy evaluations in patients\' homes.
• Develop individualized treatment plans to improve independence in activities of daily living.
• Assess home safety and recommend adaptive equipment and environmental modifications.
• Educate patients and caregivers on techniques that improve safety, function, and quality of life.
• Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
• Complete timely, accurate documentation while monitoring patient progress.
