Job summary

Home Health Care, Inc. is seeking an Occupational Therapist (OTR/L) to provide in-home therapy in the Twin Cities Metro Area. You will evaluate patients, develop personalized plans to improve activities of daily living, assess home safety, and educate caregivers and patients on safety and independence.

Qualifications

• Active Minnesota Occupational Therapist (OTR/L) license.
• Home health experience preferred but not required.
• Strong communication and organizational abilities.

Responsibilities

• Perform occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans to improve ADL independence.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on safety, function, and quality of life.
• Collaborate with physicians, nurses, PTs, SLPs and care team.
• Complete timely, accurate documentation and monitor progress.
