Responsibilities

• Evaluate patients at home and craft individualized OT plans.
• Improve daily living independence and home safety.
• Recommend home mods and adaptive equipment.
• Educate patients and caregivers on safety strategies.
• Collaborate with physicians and care team.
• Complete timely clinical documentation.
