Adara Home Health, Inc. seeks an Occupational Therapy Assistant to provide in-home OT services under the supervision of a licensed OT. You will travel to client homes, deliver 1:1 therapy, and support the plan of care with family and caregivers.

Requirements include a two-year OTA program, MN license, strong communication and teaching abilities, and reliable transportation. The role emphasizes teamwork, documentation, and adherence to policies within a dynamic home care setting.

Qualifications

• Two-year COTA program leading to an associate degree.
• Minnesota license as an Occupational Therapy Assistant.
• Strong interpersonal communication and teaching skills.
• Excellent written and oral communication skills.
• Access to a dependable vehicle or public transportation.

Responsibilities

• Provides treatments to restore and enhance the client's function under supervision of a licensed OT.
• Carries out maintenance programs developed by the licensed OT.
• Demonstrates home therapy programs to clients, families or caregivers.
• Reinforces home therapy program instructions to client and caregivers.
• Maintains documentation for the clinical record per policy.
• Discusses client progress and care plan changes with the OT.
• Participates in case conference and in-service education.
• Maintains professional communication with staff and participates in team activities.
