Job summary

Adara Home Health is seeking an Occupational Therapist to plan and provide skilled OT services in clients' homes under the Plan of Care. This role reports to the Clinical Supervisor and focuses on 1:1 client care with flexible scheduling.

The position requires Minnesota OT licensure, AOTA-approved education, and at least two years in health care; travel to multiple client homes is expected. Strong communication and organizational skills are essential.

Qualifications

• Licensed as an Occupational Therapist in Minnesota with no revocation or restrictions.
• Graduate of an AOTA-approved OT program and currently licensed to practice OT.
• Minimum 2 years healthcare experience; home care preferred.

Responsibilities

• Administer OT evaluations and develop the Plan of Care.
• Set and reassess treatment goals; update care plans.
• Provide OT treatments, procedures, evaluations and tests as prescribed by a physician.
• Instruct and supervise COTAs, HHAs, LPNs and RNs in therapy programs.
