Job summary

Adara Home Health, Inc. in Minnesota seeks an Occupational Therapist to deliver skilled therapy in clients’ homes under the Plan of Care, partnering with the Clinical Supervisor.

The role involves 1:1 visits to client homes (1–3 hours each) and requires travel within Mankato and nearby towns. Candidates should be licensed in Minnesota, have a relevant OT education, and be prepared for a flexible, team-based environment.

Qualifications

• Licensed OT in Minnesota with no license restrictions.
• Graduate of AOTA approved OT program and current certification.
• Minimum 2 years healthcare experience; home care preferred.
• NBOT certification strongly recommended.
• Strong interpersonal communication and teaching skills.
• Valid driver’s license and access to reliable transportation.
• Background checks and screenings required.

Responsibilities

• Administers OT evaluations and participates in Plan of Care development.
• Sets treatment goals and reassesses client progress.
• Provides OT treatments under physician direction and adapts activities to daily living.
• Instructs and mentors support staff and coordinates care.
• Educates families and supports home therapy program at home.
• Maintains client records and documents responses to treatment.
