Job summary

Adara Home Health, Inc. seeks a licensed Occupational Therapist Case Manager to deliver skilled therapy in clients' homes across Rochester, MN. You will plan, implement, and document care per the plan of care under supervision, with a focus on functional outcomes.

The role requires travel to home visits (1:1 care) and collaboration with clinical leadership. Compensation includes base salary with productivity bonuses and comprehensive benefits.

Qualifications

• Licensed as an Occupational Therapist in Minnesota without restrictions.
• 2+ years healthcare or home care experience preferred.
• AOTA-credentialed OT with current cert.
• Strong communication and teaching abilities.

Responsibilities

• Perform initial and periodic assessments to determine care needs and plan treatment.
• Provide OT treatments, evaluations, and prescribed procedures.
• Instruct and supervise COTAs, HHAs, LPNs, and RNs in therapy.
• Maintain records, progress notes, and client responses.
