Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to support patients in their homes, under the supervision of a licensed Occupational Therapist. You will participate in care coordination, discharge planning, and community reintegration activities while helping clients regain strength and independence.

Requirements include current OTA licensure, CPR certification, and a valid driver’s license with reliable transportation.

• Current OTA licensure in good standing.
• CPR certification current.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing rehabilitation and education programs to restore function.
• Design/adapt equipment and living environments to enhance independence.
• Deliver therapeutic treatment per physician orders to improve strength, ROM, and function.
• Document observations and progress in patient records and reports.
