Job summary

Haven Home Health & Hospice is seeking a PRN Occupational Therapist to join our home health team in Missouri. You will evaluate and treat patients in their homes, building plans of care with input from the patient, physician, and care team.

Active Missouri OT license and at least one year of recent OT experience are required; home health experience is preferred. A valid driver's license and reliable transportation are also expected.

Qualifications

• Active Missouri OT license in good standing.
• One year of recent OT experience; home health experience preferred.

Responsibilities

• Provide skilled OT evaluations and treatments in patients’ homes.
• Develop individualized plans of care in collaboration with the patient, physician, and care team.
• Help patients improve safety, independence, and ability to manage daily activities.
• Complete required assessments and documentation.
