Job summary

Active In-Home Therapy is seeking a Home Health Occupational Therapist for PRN, as-needed coverage in patients' homes. You will assess functional abilities, develop individualized treatment plans, and deliver therapy to promote independence.

As a W2 employee, you will enjoy flexible scheduling and choose areas to cover. Prior home health experience is preferred; strong clinical judgment and communication with patients and families are essential to provide high-quality care.

Qualifications

• Current state licensure as an Occupational Therapist.
• Home health experience preferred but not required.
• Strong clinical and critical thinking abilities.
• Excellent communication and interpersonal skills.

Responsibilities

• Assess patients' functional abilities to determine rehab needs.
• Develop and implement individualized OT treatment plans with the care team.
• Provide hands-on OT interventions addressing ADLs, fine motor skills, and adaptive techniques.
• Educate patients and families on adaptive strategies and devices to improve independence.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, and physical therapists for comprehensive care.
• Maintain accurate and up-to-date patient records.
• Communicate effectively with patients, families, and caregivers to ensure a supportive environment.
