Job summary

Senior Helpers of Melbourne is seeking a licensed Occupational Therapist to join our home health team in Melbourne, FL. This PRN/contract role involves delivering skilled OT services in patients' homes, evaluating needs, and creating plans of care aligned with physician orders.

You will travel throughout the service area, perform comprehensive evaluations, implement personalized treatment plans, and educate patients and caregivers on exercises and safety to promote independence.

Qualifications

• Current and unrestricted Florida Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation to travel to patients’ homes.
• Home health experience preferred but not required.
• Strong clinical judgment, communication, organization, and documentation skills.
• Comfortable working independently in a home-based clinical environment.
• Must meet all applicable background screening and employment requirements.

Responsibilities

• Perform comprehensive occupational therapy evaluations in the patient’s home.
• Develop and implement individualized plans of care based on patient needs and goals.
• Provide skilled OT treatment to improve strength, mobility, coordination, safety, and independence with activities of daily living.
• Assess the patient’s home environment and recommend adaptive equipment, devices, or modifications.
• Educate patients and caregivers regarding safety, home exercise programs, equipment use, and independence techniques.
• Monitor progress and adjust treatment plans as clinically appropriate.
• Communicate changes in condition or progress with physicians and care team.
• Complete required clinical documentation accurately and timely.
• Participate in care coordination and interdisciplinary communication.
