Job summary

Senior Helpers of Melbourne in Melbourne, FL is seeking a compassionate Occupational Therapist to provide skilled home health services. You will evaluate patients in their homes, develop individualized treatment plans, and help patients improve daily living activities.

This PRN/contract role offers flexibility to work one-on-one in patients' homes, with opportunity to grow the home health program while maintaining professional standards and completing required documentation.

Qualifications

• Current and unrestricted Florida Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and ability to travel to patients' homes throughout the service area.

Responsibilities

• Perform comprehensive occupational therapy evaluations in the patient's home.
• Develop and implement individualized plans of care based on patient needs and goals.
• Provide skilled OT treatment to improve strength, mobility, coordination, safety, and independence with activities of daily living.
• Assess the patient's home environment and recommend adaptive equipment, assistive devices, or modifications when appropriate.
• Provide patient and caregiver education regarding safety, home exercise programs, equipment use, and techniques to improve independence.
• Monitor patient progress and adjust treatment plans as clinically appropriate.
• Communicate changes in condition, concerns, and progress with the physician, Director of Nursing, and interdisciplinary care team.
• Complete all required clinical documentation accurately and within required timeframes.
• Participate in care coordination and interdisciplinary communication as needed.
• Follow all agency policies, infection-control procedures, professional standards, and applicable state and federal requirements.
