Qualifications

• Current and unrestricted Florida Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR/BLS certification.
• Valid driver’s license and ability to travel to patients’ homes.

Responsibilities

• Perform comprehensive occupational therapy evaluations in the patient’s home.
• Develop and implement individualized plans of care based on patient needs and goals.
• Provide skilled OT treatment to improve strength, mobility, coordination, safety, and independence with activities of daily living.
• Assess the patient’s home environment and recommend adaptive equipment, assistive devices, or modifications when appropriate.
• Provide patient and caregiver education regarding safety, home exercise programs, equipment use, and techniques to improve independence.
• Monitor patient progress and adjust treatment plans as clinically appropriate.
• Complete all required clinical documentation accurately and within required timeframes.

Senior Helpers of Melbourne is expanding our skilled home health services and is looking for a compassionate, dependable Occupational Therapist to join our growing clinical team. This is a great opportunity for an OT who enjoys working one-on-one with patients in their homes and wants the flexibility of a PRN or contract position while helping us grow our skilled home health program.

Position Summary

The Occupational Therapist will provide skilled occupational therapy services to patients in their homes in accordance with physician orders, the patient's plan of care, and applicable home health regulations. The OT will evaluate patients, develop individualized treatment plans, and help patients improve their ability to safely and independently perform activities of daily living.

Responsibilities

• Perform comprehensive occupational therapy evaluations in the patient's home.
• Develop and implement individualized plans of care based on patient needs and goals.
• Provide skilled OT treatment to improve strength, mobility, coordination, safety, and independence with activities of daily living.
• Assess the patient's home environment and recommend adaptive equipment, assistive devices, or modifications when appropriate.
• Provide patient and caregiver education regarding safety, home exercise programs, equipment use, and techniques to improve independence.
• Monitor patient progress and adjust treatment plans as clinically appropriate.
• Communicate changes in condition, concerns, and progress with the physician, Director of Nursing, and interdisciplinary care team.
• Complete all required clinical documentation accurately and within required timeframes.
• Participate in care coordination and interdisciplinary communication as needed.
• Follow all agency policies, infection-control procedures, professional standards, and applicable state and federal requirements.

Qualifications

• Current and unrestricted Florida Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and ability to travel to patients' homes throughout the service area.

Home Health Experience Preferred, But Not Required.

• Strong clinical judgment, communication, organization, and documentation skills.
• Comfortable working independently in a home-based clinical environment.
• Must meet all applicable background screening and employment requirements.
