Job summary

Pathways Healthcare is seeking a qualified Occupational Therapist to lead in-home rehab services in Concord, MA and surrounding areas. You will assess patients, develop individualized plans, and train patients in daily activities and adaptive equipment, coordinating with families and the care team.

Qualifications

• OT degree accredited by AMA/AOTA and NBCOT registration.
• Minimum two years of clinical OT experience preferred.
• Strong communication and organizational skills, interacting with patients and healthcare team.

Responsibilities

• Conduct comprehensive patient assessments including functional status and home environment.
• Evaluate work capacity and provide vocational training referrals as needed.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services and coordinate discharge planning.
