Job summary

Pathways Healthcare is a leading home health and hospice organization in Massachusetts, offering an in-home occupational therapy program that focuses on patient-centered recovery. The OT role supports seamless home-based care, working with a multidisciplinary team to optimize functional outcomes.

The position requires an accredited OT degree, NBCOT registration, and strong communication/organizational skills. A valid driver's license and reliable vehicle are essential for in-home visits.

Qualifications

• OT degree from an AMA/AOTA-accredited curriculum; NBCOT registration required.
• Minimum of two years of clinical OT experience preferred.
• Excellent verbal and written communication; strong organizational abilities.

Responsibilities

• Perform comprehensive patient assessments (functional status, home environment, equipment needs).
• Assess work capacity and refer for vocational training when needed.
• Develop and implement individualized OT treatment plans with reachable goals.
• Train patients in daily activities, prosthetics, orthotics and adaptive equipment use.
• Maintain accurate records and report changes in patient condition promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services and coordinate discharge planning.
