Pathways Healthcare is seeking an Occupational Therapist to join our home health and hospice team in Westwood, MA. The role involves comprehensive patient assessments, developing individualized treatment plans, and coordinating with the care team to support home-based recovery.

The ideal candidate will have OT credentials, state registration, at least two years of clinical experience, and strong communication skills. Full-time, Monday–Friday, with competitive salary and benefits.

Qualifications

• Hold an OT degree from an accredited curriculum and be registered with the National Registration Examination.
• Minimum of two years of clinical OT experience preferred.
• Excellent verbal and written communication skills and strong organizational abilities.
• Valid driver''s license and own a reliable vehicle.

Responsibilities

• Conduct comprehensive patient assessments, including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.
