Job summary

Home Caregivers Partnership is seeking an Occupational Therapist to provide intermittent home health OT services in the Gilbert, AZ area. This part-time role involves assessing patients, developing and revising plans of care, and coordinating with physicians and families.

The OT will document patient progress, complete required forms, and collaborate with the Clinical Manager to ensure quality home-based therapy. Eligible candidates hold an OT license and appropriate ACOTE/NBCOT credentials.

Qualifications

• Licensed OT in the state where practicing.
• Experience delivering occupational therapy in home health settings.
• Proficient in evaluating patients and developing/adjusting plans of care.

Responsibilities

• Assess function and develop care plans based on physician orders.
• Prepare and maintain clinical and progress notes.
• Consult with family and agency personnel as needed.
• Supervise assistants per requirements and guidelines.
• Complete OASIS assessments and other mandated documentation.
• Communicate patient progress and changes to physicians.
