Job summary

Alpha Home Health and Hospice is seeking a Licensed Occupational Therapist Assistant for per diem in-home care in Arizona. You will provide occupational therapy under supervision, guide patients and caregivers, and monitor responses to treatment, ensuring compliance with regulations.

The role requires current licensure, NBCOT certification, and a valid driver’s license or reliable transportation; CPR certification is preferred.

Qualifications

• Must hold current licensure as an Occupational Therapy Assistant.
• Must have NBOT/NBCOT certification and CPR certification (preferred).
• Valid driver’s license or reliable transportation required.

Responsibilities

• Adhere to state laws and follow the supervising OT's treatment plan.
• Instruct patients and caregivers, monitor responses, document progress.
