Job summary

St Francis LIFE Riverfront in Wilmington, DE seeks an Occupational Therapist to evaluate and treat participants in PACE programs, nursing homes, or at-home settings. The role includes home visits and occasional hospital or SNF visits to align with care plans.

Responsibilities cover comprehensive assessments, equipment management, and collaboration with the care team to enhance independence and safety in ADLs.

Qualifications

• Bachelor's degree in occupational therapy from an accredited program.
• Current, active OT license in the state of practice.
• 2 years of occupational therapist experience preferred.
• Valid driver's license and access to a vehicle.
• Strong communication and MS Office proficiency.
• Ability to plan, organize, and adapt to change.

Responsibilities

• Perform in-person comprehensive OT assessment for initial acceptance and care planning.
• Coordinate home modification assessments and equipment installation.
• Provide therapeutic treatment to achieve identified goals and collaborate with the care team.
• Assist in ordering and maintaining durable medical equipment.
• Apply OT principles to prevent functional decline and improve ADL independence.
• Train family and caregivers to maximize safety in daily activities.
• Review charts and document progress in medical records.
• Support department manager in evaluating OT staff performance.
