Job summary

HealthFlex is seeking a Home Health Occupational Therapist to support patients in California, helping them regain independence at home through personalized therapy plans and in-home interventions.

You will assess needs, design customized plans, and implement therapies to improve functional abilities in a home setting, coordinating with families and care teams. Flexible scheduling, in-house admin support, and growth opportunities are provided.

Qualifications

• CA OT license required.
• Minimum 1 year occupational therapy experience.
• AHA CPR certification required.
• Valid driver's license required.

Responsibilities

• Assess patient needs and home environment.
• Create customized therapy plans to restore independence.
• Implement home-based therapies to improve daily living activities.
• Coordinate with family and care team to track progress.
